The deadline for 2013 HEP compliance will be February 28, 2014. All required elements of HEP must be completed by this date to be considered compliant for 2013.
State Comptroller Kevin Lembo has informed us that his office has sent out letters to all HEP participants identifying compliance or non-compliance as of November. If your letter states you are compliant, retain the letter as verification. If your letter indicates non-compliance but you have already completed the item(s) indicated (or your doctor feels it is inappropriate for you complete the requirement), then there will be a “Physicians Notification” form included with your letter which can be submitted to verify compliance. The "Physicians Notification" form will be included with your letter. Submit the "Physicians Notification" form in the supplied envelope or mail to the address listed on the form.
